Documents required:
- Aadhar Card
- Identity Proof-Like Voter Card, Ration card, etc
- Permanent Residence Proof
- BPL Certificate
- Registration Certificate from Medical Organization.

Eligibility Criteria:
- Firstly, applicants should be permanent residents of West Bengal state.
- Secondly, for applying under this scheme. He/ She has not already registered with any other health-related scheme.
- An applicant who is coming under the BPL category is also eligible.
- Also, the smart card issued in the scheme is for female members of the family.
- At last, the applicant should possess all the valid details.

How to apply for Swasthya Sathi?
- First, visit the Official Link given here.
- Now at the homepage. On the top right corner, the Login option is also given above.
- As a result, the new page opened on your screen.
- then fill in the details for the new users. And then create a username and password for login.
- At last click on submit button.
- And then with the help of a username and password, you can easily log in to the official page.
Swasthya Sathi Prakalpa Registration:
- Before starting registration. Applicants have to go to the Official Link of Swasthya Sathi Scheme 2021.
- After that, the Homepage will be open in front of you.
- Then you need to click on Apply Online option given in the main menu.
- Now a drop-down list also appears below Apply Online tab.
- Then choose the fourth option given in the list. That is Form B For Registration under Swasthya Sathi Scheme.
- Then a Pdf format form appears on another page of your browser.
- After that, you need to fill in the required details. And also documents are attached with the form.
- In the end, click on the submit button. To complete the registration.
- Also, you obtain one registration number. Save it for further use.
