For births in a Government Hospital: The person can obtain the birth certificate from the Health Officer office within 21 days from the date of birth.(ব্যক্তি জন্মের তারিখ থেকে 21 দিনের মধ্যে স্বাস্থ্য আধিকারিক অফিস থেকে জন্ম শংসাপত্র পেতে পারেন।)

Births in a Private Hospital: – The person can obtain a birth certificate from the respective borough up to one year.(ব্যক্তি নিজ নিজ শহর থেকে এক বছর পর্যন্ত একটি জন্ম শংসাপত্র পেতে পারেন।)

For birth in a House: The birth certificate can be obtained from the borough, but the Head of Family should report events of birth to the concerned Ward Health Unit for Registration.(জন্ম শংসাপত্রটি শহর থেকে পাওয়া যেতে পারে, তবে পরিবারের প্রধানকে নিবন্ধনের জন্য সংশ্লিষ্ট ওয়ার্ড স্বাস্থ্য ইউনিটে জন্মের ঘটনাগুলি রিপোর্ট করতে হবে।)

Documents:

1. Proof of place of birth issued by the medical institution where the child was born.
2. Parents address  and Identity proof.

• Marriage certificate of the parent is optional.
• Declaration by the father / mother in case of live Birth. The declaration stated should be true and correct by two Gazetted Officers (the name of officers to be written in Capitals)
• Notary affidavit on (Rs. 10/. Non judicial Stamped paper)
• The original Birth certificates already taken are to be returned.
• Documentary evidences like educational certificates , election ID card, Ration card, passport, Driving Licenses, Marriage certificate, LIC policies, caste certificates, etc)
• Consent letter from concerned Hospital regarding the correction to the effect.
• Other child certificates if any.

How to apply for New Birth Certificate Online 2022 (অনলাইনে নতুন জন্ম শংসাপত্র 2022-এর জন্য কীভাবে আবেদন করবেন):

• The applicant must visit the official website of e-District west Bengal.

• Login into the e-District portal and select the service “Registration of Birth”.
• Enter the required details and upload the scanned documents.
• Finally click on submit button for successful Registration.
• Click on “Approved application” from the list of approved birth certificate.
• Now, the applicant can download the digitally signed birth certificate.

If the applicant does not receive the birth certificate, then the applicant must visit the respective Municipal Corporation and apply in person.
